Appointment Cancellations/Day of Changes Policy A minimum 72 Hours Cancellation is required or you will be charged for 100% of your scheduled appointment. This must be paid in order to schedule ANY future appointments.
NO SHOWS will automatically be charged a fee of $50 IN ADDITION TO 100% of your scheduled appointment. This must be paid in order to schedule ANY future appointments. Your online booking access may be blocked and future appointments can be denied.
*Appointments 3 hours or more require a credit card on file and a deposit of $100. No shows and missed appointments will be charged a cancellation fee of $65 per hour of scheduled appointment or 100% of the scheduled appointment, whichever is greater.
Day of Change requests must be made 72 hours prior to your scheduled appointment. Because you have booked that time for that particular service we will charge you 100% of the scheduled appointment for the lost time. Notifying us in advance, allows us the time to make appointments available for another guest needing to get in. This must be paid in order to schedule any future appointments.

Our Guest Guarantee We always strive to make your experience with us luxurious and enjoyable. If for any reason we fall short, Please make your stylist aware at the time of your actual appointment so that if time allows adjustments can be made or an appointment can be scheduled asap.
Salon Policies & Refunds sOne adjustment request is allowed but must be completed w/i 2 weeks (14 days) from original appt, for services with undesirable results.

CONSENT AND WAIVER
My signature below confirms that I have read and reviewed all items in this form. I verify by my initials and/or signatures that I understand and agree to the following terms and conditions for receiving a chemical service from Muse Parlour and representatives
• I understand that chemical treatments have different effects on different hair types and colors. I agree to hold the salon and the hair technician harmless in the event of unexpected or undesired results. I understand that I have one week to request any alterations to my color.
• I have had the process and possible results adequately explained to me. I have been advised of all options and treatments applicable to my specific needs and requests. I have been fully advised of all charges, as well as estimated costs for future visits (usually applies to color correction or total transformation services. I am aware that my desired result may require additional appointments and additional expenses. I have been given the opportunity to make requests and ask questions, as well as advise my stylist of any medications or medical conditions that could possible .
• I understand that previous treatments and chemical services can continue to have an effect on my hair. It has been recommended to me that I fully disclose my previous treatments, medications, any and all health issues that may be present and lead to contraindications at any visit to Muse Parlour and its representatives, to my stylist prior to the treatment.
• I understand that my stylist is a certified technician who will try his or her best to create my desired results. I understand that the best way to assist my technician is to be transparent about my requirements.
I am aware and understand that receiving any hair color, chemical or installation/application service can, in some individuals, cause an allergic reaction. I fully understand that this reaction can occur at anytime even if I have received this service on previous occasions. I further understand that it is policy to perform a skin patch test twenty-four hours prior to all color services. I also understand that a negative skin patch test does not mean that a reaction will not still occur. I understand these risks and if I have any concerns I will seek medical advice prior to any color service.
Further, I grant, Muse Parlour and representatives, permission to perform any and all salon/spa services (color, cut, shampoo, style, facial, waxing/epilation, permanent makeup, hair removal, hair extensions, treatments, embellishment installations, photos of services/appointment to display via various digital platforms etc.) my hair and not hold them responsible for any and all adverse health reactions or otherwise from all services.
